WHAT TO EXPECT

PREPARATION FOR YOUR VISIT

If you are a new patient, please download and print the New Patient Forms and have them ready when you come in for your appointment.  If you are an existing patient but haven't yet been seen this year, please fill out the Update Form and bring that with you to your appointment.  Please make sure you review updated office policies whether you are a new or existing patient.

We ask that you arrive 15 minutes prior to your appointment time for processing of paperwork.  If you would like for us to obtain your medical records from another practice, you may fill out the Medical Record Request form and bring it with you to the office.  If you would like for us to send your records to another party, please fill out the Medical Record Release and bring it to the office.  


​WHAT YOU NEED TO BRING:

1.  Please bring the attached New Patient Paperwork, completed and signed, to your appointment.  Or fill it out here.

2.  Please bring a valid Driver’s License / Picture ID and a current insurance card.  Be prepared to pay any specialist copayments that may apply to your visit.  Please note:  We do not accept checks.  We are able to accept cash, money order, Visa, Mastercard and Discover. You must have a picture ID in order to be seen in our office.  

3.  Please bring a complete list of all medications, including strength and dosing instructions.  For your convenience, we do offer e-prescription services and can send your prescriptions electronically to your pharmacy; simply bring your pharmacy information with you to your appointment.

4.  Please bring a copy of medical records from any previous psychiatrists, psychological testing or inpatient hospitalizations.  These records allow a better history on the patient and their condition and also provide documentation of past history should it be required by the insurance company.

 

LIFE HEALING CENTER IS CURRENTLY OFFERING TELEHEALTH SERVICES DUE TO COVID-19. IF PATIENT HAS A TELEHEALTH APPOINTMENT SCHEDULED, THEIR PROVIDER WILL SEND THEM A TEXT INVITING THEM TO JOIN THE APPOINTMENT.

CANCELLATION POLICY:

 

We respect our providers and patients time and want to accommodate everyone that we can; therefore please allow at least 24 hours notice of any cancellations or rescheduling needs.  There is a $25 charge for all late cancelled appointments and a $50 charge for all no call/no show appointments that must be paid in full to be seen.
